By bringing this completed from to your first appointment, we will be able to see you promptly.
Please notify the office before your first appointment if you have a history of a heart murmur, mitral valve prolapse, rheumatic fever, an artificial heart valve or an orthopedic implant (hip, knee, etc.).
All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
Your First Visit
We recognize that many people are at first confused and apprehensive about the prospect of periodontal treatment or implant therapy. Your questions and concerns are our number one priority and we invite you to discuss them with us at any time. At your first visit we will ask you to share with us your health history and we will perform a thorough periodontal and oral cancer examination. With your permission, we will take those X-rays necessary to make an accurate diagnosis. We will discuss our diagnosis in detail with you, along with the treatment necessary to achieve the oral health you desire.
Cancellations
Your appointment is a time reserved especially for you. If you are unable to keep an appointment, we ask that you let us know as far in advance as possible so we may give that time to another person. Please call us at 303-755-4500. Cancellations must be made 48 hours prior to your appointment to avoid a late cancellation fee.
